Switching timelines and caregiver continuity depend on your individual case and your care plan, and DHSS/DSDS — not Diverse Health Services — determines eligibility. A coordinator will confirm what applies to you.
What switching involves
Consumer Directed Services is a state program run by the Missouri Department of Health and Senior Services through its Division of Senior and Disability Services (DSDS). Participants choose who provides their CDS, and a change of vendor is a change to your care plan, so the state is part of the process. In practice there are three parts:
- 1
Tell the state and your current vendor
Changes to HCBS services are requested through DSDS's HCBS Intake and Person-Centered Care Plan (PCCP) team at 1-866-835-3505 or through the state's online change-request form. The state's page also says an HCBS provider can request a change on your behalf, and that the PCCP team calls you to review the request. Letting your current vendor know the date you intend to change keeps timesheets and payroll tidy on both sides.
- 2
The new vendor's enrolment steps
The new vendor handles its own side of the transfer with you: the vendor agreement and employer paperwork, orientation on your responsibilities as the employer of record, and the Family Care Safety Registry and background-screening steps for your attendant, which the state requires before an attendant's hours can be paid through the program.
- 3
Keeping the same attendant where program rules allow
Your attendant is your employee, not the vendor's, so a change of vendor does not by itself end that relationship. Whether your current attendant continues depends on your case and on completing the screening steps above — something we confirm with you rather than promise in advance.
Have these ready when you call
- Your current vendor's name and the date you want the change to take effect.
- Your attendant's details, so we can start the registry and screening steps on our side.
- Your most recent care plan or authorised hours, if you have a copy.
- A phone number and best time for the state's care-plan team to call you back.
Common Questions About Switching CDS Providers
What changing Consumer Directed Services vendors in St. Louis involves.
Yes. Missouri's program lets participants choose who provides their CDS, and a change of provider is requested through the state's HCBS Intake and Person-Centered Care Plan team. The exact steps depend on your care plan and current vendor, which we walk through with you directly.
Often you can keep the same caregiver when you switch vendors, since CDS is built around your choice of attendant — but it depends on your specific case and on the attendant completing our registry and screening steps, so we confirm it during your consultation rather than promising it up front.
Timelines vary by case and depend on the state's review of your change request as well as our own onboarding steps. A coordinator will give you a realistic picture once they understand your current situation.
